On May 20, 2025 LPA inspected classrooms for a complaint investigation and observed disinfecting wipes in classroom 1 within reach of children. This poses a potential health and safety risk to children in care.

Site Supervisor self-admitted that the child (C1) was left in the library, without any teacher being present for about an hour and half as child (C1) was sleeping in the library(Room 16) during morning after child was dropped off at the facility, on May 12, 2025 and based on interviews, four(4) children(C2, C3, C4 &C5) were left unattended in the bathroom on May 30, 2025, which poses an immediate risk to the health, safety and personal rights of children in care.

On May 20, 2025 LPA inspected classrooms for a complaint investigation and observed peeling paint in walls of classrooms 1, 4, and 5B. LPA also observed a dead fly and other insects on the windowsill of classroom 8. Classroom 2 sink was leaking water on floor making it a slipping hazard. This poses a potential health and safety risk to children in care.
