Based on record review the licensee did not comply with the section cited above in four persons. Licensee is not documenting the times she check on infants at sleep, for children ch1, ch2, ch3, ch4, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall start documenting every 15 minutes when infants sleep starting on 6/14/23 and until children reach the age of 2 years old. Licensee will submit to Licensing Program the log of documenting infants sleep for the children ch1, ch2. ch3, ch4 for the period 6/14 to 6/23/23 and will submit to licensing not later than 6/27/23.
Based on record review the licensee did not comply with the section cited above in two cases, Licensee has not obtained a signed lic9227 for two infants (ch 1, ch2) which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall complete and obtain signed forms LIC9227 for infants ch1 and ch2. Licensee will submit to Licensing program copies of forms LIC9227 for those 2 infants not later than 6/27/23
