The Site Director did not ensure that staff:child ratio requirements were met, as staff reported supervising more than 14 children, which poses an immediate risk to the health, safety, and personal rights of children in care.

Based on interview and record review, the licensee did not comply with the section cited above as it has been more than 180 days since the facility received a license, which poses a potential health, safety or personal rights risk to persons in care.

Site Director present at the facilty, Stephanie Stellman, was associated to the facility on 1/8/2023, and the department was not notified of changes which poses a potential threat to the health, safety, and personal rights of children in care.

The facility was notified about a child being left outside unattended on 1/3/2024 and the initial report to the Department was made on 1/8/2024, which poses a potential risk to the health, safety, and personal rights of children in care.

The licensee failed to maintain personnel records as many required documents were missing which poses a potential threat to the health, safety, and personal rights of children in care.

Four out of seven personnel files reviewed did not have proof physicians report, which poses a potential risk to the health, safety, and personal rights of children in care.

(f) When the child care center director is absent from the center, arrangements shall be made for a fully qualified teacher as specified in Section 101216.1(c) to act as substitute. During winter camp hosted at Vine Hill, staff from Brook Knoll were assisting without fingerprint clearance transfer between the facilities, which poses a potential risk to the health, safety, and personal rights of children in care.

The Site Director did not provide proof of required immunizations for two out of seven personnel files reviewed, which poses a potential risk to the health, safety, and personal rights of children in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above as three out of three medications stored at the facility were expired, which poses a potential health, safety or personal rights risk to persons in care.

When the Site Director left the facility at 4:00PM on the day of the incident, she did not designate a fully qualified teacher to act as a substitute director, which poses a potential risk to the health, safety, and personal rights of children in care.
