Based on observations and interviews, it was determined that during a parent event at the end of the day, child, C1 ran out of the two facility entrance gates, outside the center and down the sidewalk parallel to the left side outdoor area. This poses an immediate risk to the health, saftey and personal rights of childrne in care.
Based on observation, interview and record review, the licensee did not comply with the section cited above for one staff, S3 which poses a potential health, safety or personal rights risk to persons in care. Staff, S3 was observed to be missing several documents including proof of immunization's and Mandated Reporter Training.

LPA observed that there is not gate or door to make the kitchen inaccessible. There are knives in a drawer that is accessible to children.
A child left the playground and entered the classroom with out a Teacher being present in the classroom. This is a zero tolerance violation. This poses a immediate risk to the health and safety of children in care.
