Based on observations and interviews, it was determined that during a parent event at the end of the day, child, C1 ran out of the two facility entrance gates, outside the center and down the sidewalk parallel to the left side outdoor area. This poses an immediate risk to the health, saftey and personal rights of childrne in care.

Staff, S3 was observed to be missing several documents including proof of immunization's and Mandated Reporter Training.
