Based on observation, interview and record review, the licensee did not comply with the section cited above for herself which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to complete the Pediatric CPR/ First Aid training and submit certificate to LPA by 08/15/2025.

Based on observation the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPAs observed one infant (C2) sleeping in a play yard with a blanket. Child was observed to be asleep in the master bedroom with the door closed while Licensee was in the covered patio with other children.

Resolution: Licensee will submit a written letter to CCLD explaing that she understands safe sleep regulations and that doors must remian open when children are sleeping in another room by POC Date.

Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensee's Mandated Reporter Training expired on 12/16/2023 and has not been renewed.

Resolution: Liceense will submit to CCLD a picture of her updated Mandated Reporter Training certificate by POC date.

Based on record review the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. One infant (C1) was observed to be missing the Individual Infant Sleep Plan (LIC 9227)

Resolution: Licensee will submit a LIC 9227 for child, C1 to CCLD by POC Date

Based on observation, interview and record review, the licensee did not comply with the section cited above for two infants, C1 and C2 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will begin to do 15 min checks on infants under two years and will submt a sleeping log to CCLD by POC date

Based on observation, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. LPAs observed cleaning agents, laundry detergent, scissors, car cleaner and pet shampoo under the bathroom sink near the outdoor play area that is accessible to children. LPAs also observed unopened bottles of alcohol in lower shelf of the cabinet in the covered patio.

Resolution: Licensee will send photos to CCLD within 24 hours of cleaning compounds and other hazards removed or made inaccessible to children.

Based on observation the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. LPAs observed one infant (C2) sleeping in a play yard with a blanket. Child was observed to be asleep in the master bedroom with the door closed while Licensee was in the covered patio with other children. LPAs also observed a preschool child asleep in a play yard in the covered patio with a pillow and blanket inside it.

Resolution: Licnesee will send a written statement to CCLD within 24 hours explaining she understands the safe sleep regulations and will keep all loose articles out of play yards and cribs.

Based on observation and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a photo to CCLD by POC date showing that they have conducted a fire drill.

Based on observation the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPAs observed a baby bouncer in the on limit covered patio.

Resolution: Licensee will submit a picture that the baby bouncer is put away and no longer in use by the children.

Per LPA's review of files, Licensee's two helpers do not have Mandated Reporter Training per AB1207. Licensee stated she was unaware of the requirement. This poses a potential risk to the health and safety of children in care.

Resolution: BY POC DUE DATE Licensee's two helpers will complete the training and send a copy of the Certificate of Completion to Licensing Office. LPA provided the web address https://www.mandatedreporterca.com to access the required training.

Based on records and records review , the licensee did not comply with the section cited above in 2 out of 2 helpers, S1 and S2, did not have files at the facility which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: By POC DATE 11/16/2022 Licensee will provide complete copies of required documentation in a file to the Licensing Office.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above because Licensee Aljbana Kraja had a total of 5 infants and 2 preschoolers with no assistants, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: BY POC DUE DATE Licensee agreed to submit a written plan of action regarding how Licensee will ensure that facility is in compliance with Title 22 Regulation Section 102416.5(d)(1). A copy of this Licensing Report must be given to each existing parent and to the parent of children who are enrolled over the next 12 months. In addition, a copy of the LIC 9224 Acknowledgement of Receipt of Licensing Reports must be signed by each parent and be kept in each child's file.

Per LPA's review of Licensee's file, the Licensee Aljbana Kraja does not have current Pediatric CPR/First Aid certification on file. This poses a potential risk to the health and safety of children.

Resolution: BY POC DUE DATE: The Licensee agrees to provide proof of CPR/First Aid completion to Licensing office

Based on LPA's observation, interview and record review, the licensee did not comply with the section cited above in the file review out of Licensee Aljbana Kraja. This poses/posed a potential health, safety or personal rights risk to persons in care.
