Based on observation, interview, record review, the licensee did not comply with the section cited above as evidence by the four infants under 24 months and only one shool-age child, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure they are abiding by licensed requirements for ratio and capacity and will review the ratio and capacity handout provided by the department
Based on LPA's observations and record review, the licensee did not comply with the section cited above in 1:1 staff records did not have a current mandated reporter training certificate. This poses a potential health and safety risk to persons in care.
Resolution: Licensee states that they will complete the mandated reporter training and will submit certificate of completion to LPA by poc date via email.
