Based on observation, the licensee did not comply with the section cited above.a basket of over the counter medication, all of which appeared to be natural products. At least one of the products, "ColdCalm" label stated "Keep out of reach of children." The basket was located on a low shelf located below the changing table in family room. Two mobile infants were observed in the family room during today's inspection with access to the medication, which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a statement of her understanding of the regulation and how to safely store medications. The statement will be emailed to LPA Strother at amy.strother@dss.ca.gov

Based on observation and interview, the licensee did not comply with the section cited above. At 9:23am LPA observed a netted dome attached to a play yard in the Master Bedroom. LPA asked L1 if the play yard is used by day care children. L1 stated yes, it is used by C2, an infant in care during today's inspection and also by a two year, not present, who is able to climb out otherwise, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the netted dome from the play yard and put it in storage during today's visit. Licensee stated will submit a written statement of their understanding of the infant safe sleep regulations.

Based on observation, the licensee did not comply with the section cited above. At 10:29am LPA went over to the room infant C1 was sleeping in to check the condition of the sleeping environment. L1 had completed a safe sleep check at 10:24am. LPA observed the door to be slightly open, not wide enough to see the sleeping infant, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a written statement of L1 and L2 understanding of the infant safe sleep regulations. by 11/27/25 to email: amy.strother@dss.ca.gov

Based on observation and interview, the licensee did not comply with the section cited above in that the off-limits detached structure, labelled Toddler Room in the back yard, does not have a fire clearance for use was observed to be unlocked, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will submit a statement of L1 and L2's understanding that the door to the Toddler Room must remain locked at all times during child care hours and how this will be accomplished, by 11/27/25 to email: amy.strother@dss.ca.gov

Based on interview, the licensee did not comply with the section cited above in one out of one infant (C4) in care. Licensee stated that C4 sleeps in an adult bed with C4's mom or on the bed alone or in a providers ares, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will change the room C4 sleeps in to a room that is not C4's room and have C4 sleep in a play yard or crib beginning 12/10/24. L1 will submit a written statement with her plan for C4's sleeping environment.

Based on interview, the licensee did not comply with the section cited above. L1 stated that infant C4 sleeps in C4's room with the door closed with or without C4's parent present and is not supervised by a provider while sleeping, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will begin observing C4 and all infants while sleeping beginning 12/10/24. L1 will provide LPA with a written statement on how she will comply with the regulation to supervise infants while sleeping.

Based on interview and record review, the licensee did not comply with the section cited above in 3 out of 5 files reviewed. Children C1, C4 and C5 did not have all of the required immunizations on file, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will obtain immunizations for children C1 and C5 transcibing the records onto the CDPH286 form and send completed records to LPA Strother, amy.strother@dss.ca.gov by 12/16/24. L1 stated that she will work with C4's parent to obtain a medical exemption through, CAIR-ME, providing a statement from a doctor that the exemption will be granted by 12/16/24. L1 will provide a statement for an alternative plan, if a medical exemption is not granted for C4.

Based on interview and record review, the licensee did not comply with the section cited above for one infant (C4) in care. L1 stated that she has not completed safe sleep logs to include date, name of infant and the time of each 15 minute check for C4, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: L1 stated she will begin Sleep Logs for infants in care on 12/10/24 and send copies of sleep logs to LPA on 12/16/24. amy.strother@dss.ca.gov

SERIOUSHEALTHCCR 102423(a)(2)Type BDec 9, 2024

Based on observation and interview, the licensee did not comply with the section cited above for two children in care (C2 and C3). C2 and C3, were observed to be sleeping in sleep sacks inside of play yards. L1 stated that C2 and C3 (both over the age of 2 years) are able to climb out of the play yard. Manufacturer instructions are to discontinue use when a child is able to climb out and when a child is 35 inches in height, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: L1 stated she will have C2 and C3 sleep on play mats in the living room beginning 12/10/24. L1 will provide a written statement that she understands that once children can climb out of play yard, she must dicontinue use for those children. Submitting the statement to LPA Strother's email

CRITICALHEALTHCCR 102371(a)Type ADec 9, 2024

Based on interview and record review, the licensee did not comply with the section cited above. On 01/30/24 the facility's request for a fire clearance to add a room labelled "toddler room" to the approved area was denied. The toddler room is an unattached building located in the back yard. During an interview with Licensee (L1), L1 stated at 2:00pm that they have been using the room to provide care, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: License locked the door to the "toddler room", stating that it will not be used until a fire clearance is granted. Licensee provided form LIC9054 and the facility sketch LIC999A to LPA during today's visit to request an updated and approved fire clearance that will not include the now marked "off limits" toddler room.
