Based on observation, LPA observed child 1 (C1) sleeping in a playpen in bedroom 1 with a blanket which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee removed the blanket from the playpen and LPA reviewed the Infant Safe Sleep regulations with the Licensee who submitted a signed statement attesting to her understanding of the regulations to LPA.
Based on record review, the licensee did not record sleep checks for child 1 (C1) and child 2 (C2) for every day they slept at the facility which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she would record sleep checks for all infants in care and send LPA a one week of sleep checks by email at robert.maciel@dss.ca.gov.
Based on record review, the licensee did not possess a current Mandated Reporter Training Certificate which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she would obtain a current mandated reporter training certificate and send a copy to LPA by email at robert.maciel@dss.ca.gov
Based on record review, child 1 (C1) did not possess a complete LIC9227 Individual Infant Sleepig Plan which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she would obtain a complete LIC9227 form from C1's parent and send a copy to LPA by email at robert.maciel@dss.ca.gov.
Based on record review, child 5 did not possess complete immunization records which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she would complete the immunization record and email a copy to LPA at robert.maciel@dss.ca.gov.
