Based on observation, the licensee did not comply with the section cited above. At 10:00am LPA observed a small blanket with "lovey" stuffed animal head and a thin baby blanket in the play yard with infant C5 which posed a potential health, safety or personal rights risk to persons in care.

Resolution: The loose items were immediately removed from the play yard during today's visit. Licensee will submit a written statement outlining her understanding of the infant safe sleep regulations, how they will be implemented and how she will ensure that assistants are also aware of the regulations.

Based on observation, the licensee did not comply with the section cited above in 2 out of 2 staff files. Both S1 and L1 have expired Mandated Reporter training certificates and have not completed the Mandated Reporter training in the last two years, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: L1 stated that she and S1 will complete the Mandated Reporter Training for Child Care Providers on the www.mandatedreporterca.com website and submit the updated certficates to LPA Strother's email: amy.strother@dss.ca.gov by 02/20/26.

Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 2 staff files. S1, and assistant who was caring for children without the Licensee present, did not have a current training certificate and had not completed the training within the last two years, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will work with S1 to schedule an approved Pediatric First Aid/CPR course and will not be left alone with the children until S1 has completed the course, submitted the certifcate of completion to LPA Strother by 03/16/26. Licensee will also submit a written statement with a plan on how she will track the expiration of training certificates to ensure they remain current.
