Based on observation the licensee did not comply with the section cited above. At 2:15pm LPA observed four large black storage bins with yellow lids stacked one on top of the other. LPA pushed gently on the storage bins and observed them wobble, indicating that they were not stable and could fall over if pushed or run into with force. There were other various bins stacked up high along the fence with a large stroller in front of them that were also accessible to children. At 2:29pm LPA observed a total of 11 uncovered electrical outlets in the home, 3 in the living room, 1 in the hallway and 7 in the dining room, which posed a potential health, safety or personal rights risk to persons in care.

Resolution: L1 put safety plugs in all 11 outlets during the inspection. L1 will submit a written statement that she has discussed with all family members the importance of replacing outlet covers when removed. L1 will remove all bins out of the children's area into an area that is not accessible to children. L1 will submit photos of the outdoor area once it is cleared, no later than 07/17/25. Based on observation, the licensee did not comply with the section cited above. At 1:40pm, the licensee did not comply with the section cited above in for two out of four infants present. Two infants, C3 and C6 were observed sleeping in a double stroller in the living room which posed a potential health, safety or personal rights risk to persons in care.

Resolution: L1 will submit a written and signed statement with a plan for putting infants down for nap times, to include where they will be sleeping. amy.strother@dss.ca.gov

Based on record review and interview, the licensee did not comply with the section cited above in 1 out of 6 children's files reviewed. Child C3's file did not contain current immunization records, C3 was missing 5 required immunizations, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: L1 will request documentation of current immunizations from C3's parents, update the CPDH286 (blue card) and submit a copy to LPA Strother.

Based on interview and record review, the licensee did not comply with the section cited above in 4 out of 4 infant files. LPA observed the last log was dated 06/25/25. Licensee stated that she has not completed logs for sleep checks for the last two weeks, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: L1 will document sleep checks every 15 minutes beginning tomorrow and send a week of safe sleep log documentation to LPA Strother.
