Based on interview and record review, the licensee did not comply with the section cited above in 3 out of 3 staff files. The licensee (L1) and two assistants (S1 and S2) do not have current Mandated Reporter Training certificates on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete and ask assistants S1 and S2 to complete the General and Child Care Provider Mandated Reporter Training at mandatedreporterca.com and email completed certificate copies to LPA Strother at amy.strother@dss.ca.gov
Based on interview and record review, the licensee did not comply with the section cited above in 3 out of 5 children's records. Children C1, C4 and C5 were missing some of their required immunizations on record which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee (L1) messaged the authorized representatives of children C1, C4 and C5 during the inspection to request updated immunization record print outs. L1 will work with the families to obtain current records, and transcribe updated immunizations to "blue card" CDPH286, sending copies to LPA Strother at amy.strother@dss.ca.gov.
