Resolution: Licensee will provide proof of completion of the required Mandated Reporter training for herself and staff no later than Monday April 14, 2025. Licensee will ensure that the requirement is met every 2 years.

Resolution: Licensee stated that she will go to her Personal Doctor Doctor because she know she has had all of the immunizations and will have a blood test done. For thestaff will be the same.
