LPA met with Assistant Director, Rakam Dabash, and toured the facility indoors and outdoors. Days and hours of operation are Monday – Friday 7:30 a.m. – 6:00 p.m. There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. No poisons were observed during the inspection. There was fully charger Fire Extinguisher in both classrooms. Classrooms #1 is not currently in use. Classroom #2 and Classroom#3 is currently using for childcare. LPA toured all three rooms. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. Playground equipment is in safe condition, free of sharp, loose, or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the classrooms are clean and safe. All food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Facility is located on a school district site and food is provided by the school district. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors. Water cooler system was observed in classroom #2 and #3. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. The facility is free of flies, insects, and rodents. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. Facility has an integrated fire life safety system with carbon monoxide included. LPA was able to check a fully first aid backpack that is in each classroom.Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care. LPA observed in classroom#2 a total of 13 children and 2 teachers. In classroom#3 LPA observed 12 children and 2 teachers. LPA reviewed a sample of seven children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA was able to check three children that are on medication. LPA was able to review the Emergency Care Plan, Medication at school form, and Medication documentation forms. Assistant Director Reham Dabash shared with LPA that the staff Core Files are located at the District Office. LPA was able to review the Child Development Service Employee Files. Staff are current on their Mandated Reporter Training and First Aid and CPR.LPA reviewed a sample of seven staff files and observed files were complete with current documentation of completed mandated reporter training and CPR/ First Aid. Staff documents containing employee rights, and health screening reports are kept at the District Office due to confidentiality. Menus are posted at least one week in advance where an authorized representative can view them.
