Based on interview and record review, the licensee did not comply with the section cited above in not having documentation of a current drill log, with the last drill documented on 10/01/2022, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will conduct and document a fire drill no later than 02/19/2026 and submit proof of documentation to LPA Billones via email at bill.billones@dss.ca.gov. Licensee will also ensure drills are being conducted at least once every six months moving forward.
Based on record review, the licensee did not comply with the section cited above in 1 out of 6 children's records reviewed, which poses a potential health, safety or personal rights risk to persons in care. LPA observed Licensee did not maintain sleep charts for one children's file for Child 6 (C6). C6's last entry was in August 2025. C6 was napping during part of today's inspection.
Resolution: Licensee will ensure infants 24 months and under have proper documention of 15-minute sleep checks. Licensee will read Title 22 Infant Safe Sleep regulations and sign an acknowledged stating they understand the importance of proper sleep documentation. Licensee to submit acknowledgement to LPA Billones via email provided no later than 02/19/2026.
Based on record review, the licensee did not comply with the section cited above in 2 of 2 staff records reviewed, which poses a potential health, safety or personal rights risk to persons in care. LPA observed Staff 1 (S1) did not have a current certificate on file and Staff 2 (S2) did not have a certificate on file.
Resolution: Licensee and staff will take the Mandated Reporter training course from a valid vendor no later than 02/19/2026, and submit the certificates of completion to LPA Billones via email provided.
