SOTO FAMILY CHILD CARE

Based on observation, interview, and record review, the licensee did not comply with the section cited above in not having a current flu vaccination for her self (licensee) which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will explain in writing how she will ensure to remain current with immunizations. The POC will be sent to LPA Mendez. Business card is left for convenience. Licensee will submit by the end of business day 12/15/2025.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in C1 is missing immunization records which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licnesee will submit a letter detailing how she will remain in complience with regulations and required documentation. POC will be sent to LPA Mendez by the end of business day 12/15/2025. Business card left for convenience.

Based on observation and interview, the licensee did not comply with the section cited above in shampoos and conditioners are within children's reach which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will explain in writing how she will secure poisonous and hazardous items from the accessible areas of children. The POC will be sent to LPA Mendez. Business card is left for convenience. Licensee will submit by the end of business day 12/08/2025.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in last fire drill conducted over a year ago 9/04/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will explain in writing how she will ensure to regulary conduct the required drills. The POC will be sent to LPA Mendez. Business card is left for convenience. Licensee will submit by the end of business day 12/15/2025.

Based on observation, the licensee did not comply with the section cited above in an infant was sleeping in a carseat upon arrival to the FCCH which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide a letter of how she will prevent this from happening again in the future by email to LPA Mendez at joaquin.mendez@dss.ca.gov. This will be sent by the end of business day on 9/13/2024

Based on interview and record review, the licensee did not comply with the section cited above in one (1) S1 did not have the mandated reporter training certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide a copy of certificate by email to LPA Mendez at joaquin.mendez@dss.ca.gov. This will be sent by the end of business day on 9/13/2024

Based on LPAs observed in the children's bathroom and kitchen, oral rinse, personal hygiene, and a knife accessible to children in care, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will email photos of the oral rinse, personal hygiene items and knife inaccessible to children in care by July 20, 2022, via email: Martina.Jimenez@dss.ca.gov

Based on observation and interviews with the assistant and licensee's husband who stated that the licensee has been absence from the FCCH since Saturday, July 16, 2022. Licensee went to Jalico to visit her father, and is due to return on July 20, 2022, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will submitted a written statement on how will prevent future incidents, how licensee shall be present in the home and licensee's temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day by 7/20/2022, via email: Martina.Jimenez@dss.ca.gov

Based on LPAs observation, interview, and record review, revealed that the licensee and assistant have not completed the Mandated Reporter Training required per AB 1207, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit verification of completed mandated reporter training certificate for licensee and assistant to CCLD by 7/26/2022, via email: Martina.Jimenez@dss.ca.gov

LPAs observation, interview, and record review revealed that Licensee and Assistant, Blanca Maldonado, did not have verification of immunization's, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit verification of Blanca Maldonado's immunization to CCLD by 7/26/2022, via email: Martina.Jimenez@ds.ca.gov

Based on observation, interviews with the assistant and children, the licensee did not comply with the section cited above in that Licensee is using the enclosed patio for the purpose of napping and meal/snacks, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will move the eating and napping area to the inside of the home. Licensee will submitted photos of corrections and submit a written statement on how licensee will prevent future incidents from happening in the future to LPA Jimenez, via email: Martina.Jimenez@dss.ca.gov by 7/26/22.
