Based on observation and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed a fire extinguisher that did not have a sevice ticket or a purchase receipt. Licensee acknowleged there was no service ticket or receipt avaliable for review.

Resolution: Licensee to service existing fire extinguisher or purchase a new regulation fire extinguisher (2A10BC or above). Licensee to submit documented proof of correction to Licensing Program Analyst Bill Billones via email at bill.billones@dss.ca.gov no later than 8/20/2025.

Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA reviewed staff records and observed licensee's mandated reporter training expired on 6/8/2024.

Resolution: Licensee and any volunteers and/or employees of the FCCH to renew mandated reporter training and submit certificate(s) to Licensing Program Analyst Bill Billones via email at bill.billones@dss.ca.gov no later than 8/20/2025. LPA advised licensee mandated reporter training must be renewed every two years.

Based on interview and record review, the licensee did not comply with the section cited abovewhich poses a potential health, safety or personal rights risk to persons in care. LPA requested the children's roster and licensee stated they did not maintain a roster on file.

Resolution: Licensee to update and maintain a facility children's roster in the FCCH. Licensee to submit documented proof of correction to Licensing Program Analyst Bill Billones via email at bill.billones@dss.ca.gov no later than 8/20/2025.

Based on record review, the licensee did not comply with the section cited above in 3 out of 5 children's files reviewed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to ensure compliance with Title 22 regulations by maintaining required licensing forms or corresponding documentation is kept in each children's file. Licensee to provide a written statement acknowdleding they have read and understand the requirements of Title 22 Regulations and submit to Licensing Program Analyst Bill Billones via email bill.billones@dss.ca.gov no later than 8/20/2025.
