Based on observation, interview, and record review, the licensee did not comply with the section cited above in the licensee and S1 are missing the mandated training reporter training certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above in a baby walker is in the accessible area to children, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licnesee will provide a letter stating how she will prevent banned items in the accessible area to children. Business card with contact information provided.

Based on observation and interview, the licensee did not comply with the section cited above in various chemicals, medicines, and hygiene products are found accessible, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licesnee immediately removed items to an inaccessible area. Licensee also locked bedroom door preventing access. Licensee will write an explanation on what she will impliment to prevent this from happening in the future. Business card with contact information provided.

Based on observation, interview, and record review, the licensee did not comply with the section cited above the licenssee could not provide a purchase reciept for her fire extinguisheer which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send a copy of the purchase receipt to the LPA. Business card with contact information provided.

Based on LPA observed a large room additional attached to the home and currently under construction. The licensee advised LPA that licensee did not notify CCLD of the room additional nor did the licensee obtain building permits for the room addition, the licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit verification of permits and a written statement on how licensee will prevent future incidents to CCLD by 3/4/2024, via email to: Martina.Jimenez@dss.ca.gov

Based on LPAs observation, interview with licensee, revealed that the licensee did not comply with the section cited above in assuring the following items are inaccessible the children in care; ladders, tool boxes, tools, drills, pliers, and construction materials were observed in the children's play area and accessible to children in care at the time of the inspection, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit photo of all ladders, tool boxes, tools, drills, pliers, and construction materials in the children's play area accessible to children in care are made inaccessible to the children's in care. Licensee will submit how licensee will provide future incidents from occurring to CCLD by 2/27/2024, via email to: Martina.Jimenez@dss.ca.gov

Based on LPAs observation and interview licensee, revealed the licensee did not comply with the section cited above in posting or completing the emergency disaster plan in the FCCH, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPAs observation and interview with the licensee who stated the emergency disaster drill was last conducted on August 3, 2022, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPAs observation on the kitchen counter Clorox disinfecting wipes, glass cleaner and dog feces in the backyard accessible to children in care. Which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee to submit to CCLD by 1/12/22, verification of correction (photos) as well as written statement noting plan of correction to prevent future occurrences.

Base on LPAs observation, interviews and record review revealed that Licensee and Assistant were unable to provide the mandated reporter certification, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and Assistant will complete the Mandated Reporter Training, and submit verification of completion to CCLD by January 18, 2022
