Based on observation and interview, the licensee did not comply with the section cited above in having adult vitamins, Vick's Vapor rub, lotions, and various hygiene products within children's reach which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee to submit to CCLD by 11/24/2025, a written statement noting plan of correction to prevent future occurrences. Business card with contact information is provided to the licensee.
Based on observation and interview, the licensee did not comply with the section cited above in a fire place in the living room is missing a door and is accessible to children which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee to submit to CCLD by 11/24/2025, verification of correction (photos) as well as written statement noting plan of correction to prevent future occurrences. Business card with contact information is provided to the licensee.
Based on record review the licensee did not comply with the section cited above Licensee's adult son who turned 18 years old in January 10, 2022 has no criminal record clearance at the time of the inspection which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: On the same day, 7/28/2022, Adult 1 attempted to obtain a criminla record clearance (CRC) but was given an appointment to go back. . Adult 1 will resort to different agencies to obtain the CRC. Licensee will submit a written plan of correction to ensure that all adults prior to living, working in the FCCH are fingerprint cleared. A civil penalty of $ 500.00 will be assessed.
