Based on observation and interview , the licensee did not comply with the section cited above in having the infant present napping in a crib with a blanket which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will review Safe Sleep regulation and FAQs provided by LPA and submit a statement that they reviewed and understand the regulations. Statement will be provided to LPA via email at giovani.gonzalez@dss.ca.gov no later than 2/12/25

Resolution: Licensee will complete and submit an updated Mandated Reporter certificate to LPA no later than 2/12/25 via email at giovani. gonzalez@dss.ca.gov.
Based on interview and record review, the licensee did not comply with the section cited above in licensee not being able to locate file/documents for their assistant which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide a copy of assistant's complete file to LPA via email at giovani.gonzalez@dss.ca.gov no later that 2/12/25.
