Based on record review, the licensee did not comply with the section cited above, Infants enrolled under 12 months do not have Infant Safe Sleep Plan on file (LIC 9227)] which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit a plan of correction outlining how she will ensure that Infants under the age of 12 months have a completed LIC 9227 or Infant Safe Sleep plan upon enrollment. Submit POC no later than 6/30/2025.
Based on record review, Licensee did not comply with the section cited above, 2 children C9 and C17 were enrolled at the day care without immunization which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit a plan of correction outlining how she will ensure that children are immunized prior to being admitted at the day care. Submit POC no later than 6/30/2025.
Based on record review, , the licensee did not comply with the section cited above in 5 out of 20 children present in the home do not have available files which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit a plan of correction outlining how she will ensure that all day care children must have complete file in the FCCH. Submit POC no later than 6/30/2025.
Based on interview and record review , the licensee did not comply with the section cited above FCCH does not have current children's roster which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit a plan of correction outlining how she will ensure that day care children have files available at the FCCH Submit POC no later than 6/30/2025.
Based on observation, the licensee did not comply with the section cited above, there were 5 infants, 7 older children and 8 school age children present in the home which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee started calling the parents of the day care children and requested that they pick up their children from the facility. Licensee agreed to attend an Infomal conference upon invitation. the Licensee also agreed to submit a written Plan of Correction outlining how the Licensee will prevent the instances of over capacity.
Based on record review, the licensee did not comply with the section cited above, licensee stated last drill was conducted on 5/22/2025, however, based on disaster drill log, the last drill was conducted in 2022, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit a written plan of correction on how to ensure that fire and disaster drill are conducted once every six months and documented. POC letter must be submitted no later thatn 6/30/2025
Based on record review and interview, the licensee did not comply with the section cited above , the licensee and assistant are not properly and consistently documenting napping infants which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit a plan of correction on how to ensure that 15-minute check of napping infatns are properly documented. POC must be submitted no later than 6/30/2025
Based on record review, the licensee did not comply with the section cited above in Licensee and Assistant 1 do not have current Mandated Training Certificate on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit the current certificates and POC letter on how to ensure that Mandated Reporter Training are renewed every to years. POC shall be submitted no later than 6/302025
Based on observation, the licensee did not comply with the section cited above in at arrival assistant was providing care and supervision to eight (8) children all under the age of 5 years which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agree to submit a written plan detailing how she plans to ensure that she will be within her capacity at all time and will submit via email to Francisca.Velazquez@dss.ca.gov by 6/12/2023.

Resolution: Licensee agrees to make sure that assistant completes mandated reporter training certificate and submit proof of completed certificate via email to Francisca.Velazquez@dss.ca.gov by 6/12/23.

Resolution: Licensee agrees to make sure individual is fingerprinted and clears and agrees to not have assistant present until individual is cleared. Licensee will submit proof of livescan via email to Francisca.Velazquez@dss.ca.gov by 6/6/23.
