Based on observation, interview, and record review, the licensee did not comply with the section cited above in [1] out of [1 count] of changes to the home with ut notifying the Department which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The Licensee will send an updated LIC279 and include the changes on the LIC999 to the LPA. The licensee will include a POC letter stating how she will in the future remeber to notify the department prior to making changes to the FCCH.
