Based on record review and interview the licensee did not comply with the section cited above since 1 assistant's most recent Tb clearance was in 12/2021 annd the assistant began in 02/2024 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide proof of current Tb clearance to LPA via email at giovani.gonzalez@dss.ca.gov, no later than 4/16/2025.
Based on observation and record review there were five (5) children being cared for by one adult (four which were infants younger than two (2) years old and one which was older than 3 yeras old) which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit a written summary explaining how she will ensure that when there is one adult present only, how capacities will be maintain. Licensee will submit written summary via email to francisca.velazquez@dss.ca.gov by 8/30/22.
