Based on interview and record review, the licensee did not comply with the section cited above in that 15-minute sleep check logs were found to be incomplete for Child #04 (C4), which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit to the Department (seena.parsapour@dss.ca.gov) a written plan explaining how they plan on ensuring compliance with this regulation moving forward, no later than October 31st, 2025.
Based on interview and record review, the licensee did not comply with the section cited above in that the Individual Infant Sleeping Plan (LIC 9227) for C4 was not on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
