Based on observation, the licensee did not comply with the section cited above in that a changing station was affixed to the top of the "pack 'n play" style crib located against the wall next to the television in the living room area, with Child #01 (C1) inside of the crib at the time of observation, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit to the Department (seena.parsapour@dss.ca.gov) a written statement explaining how she plans on ensuring compliance with this regulation moving forward, no later than October 17th 2025.
Based on interview and record review, the licensee did not comply with the section cited above in that currrent AB1207 compliant Mandated Reporter Training Certification was not on file for an assistant present at the time of the inspection (the licensee's father-in-law), which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit to the Department (seena.parsapour@dss.ca.gov) proof of current AB1207 compliant Mandated Reporter Training Certification for the aforementioned assistant, as well as a written statement explaining how she plans on ensuring compliance with this regulation moving forward, no later than October 17th 2025.
