Based on observation, the licensee did not comply with the section cited above in upon arrival licensee had infant under 12 months swaddled and asleep on the couch which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee removed the infant from the couch during the inspection. Licensee to submit a written statement on how facility will maintain infant safe sleep regulations in the future which can be emailed to elizabeth.george@dss.ca.gov no later than January 6, 2026.
Based on observation, the licensee did not comply with the section cited above in infant under 12 months was asleep on the couch upon LPAs arrival which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee removed the infant from the couch during the inspection. Licensee to submit a written statement on how facility will maintain infant safe sleep regulations in the future which can be emailed to elizabeth.george@dss.ca.gov no later than January 6, 2026.
Based on record review, the licensee did not comply with the section cited above in one out of one infant files did not have the LIC 9227 Individual Infant Sleeping Plan completed which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee to obtain a LIC9227 Individual Infant Sleeping Plan for all children under 12 months of age. Submit proof of paperwork obtained to elizabeth.george@dss.ca.gov no later than January 9, 2026.
