Based on observation, the licensee did not comply with the section cited above, there is a 2A10BC fire extinguisher however, there is no proof of purchse nor service card which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to purchase a new fire extinguisher or to have it serviced and submit the plan of corection to CCL no later than 1/13/2023.
Based on interview and record review, the licensee did not comply with the section cited above, licensee stated she is checking napping infant but there was no documentation which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to document the following as she checks the napping infant every 15 minutes as well as any eventful situation. a. Date. b. Infant’s name. c. Time of each 15-minute check.
Based on record review the licensee did not comply with the section cited above in Licensee's assistant does not have immunization record on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to submit the assistant's immunization record to Community Care Licensing (CCL) no later than 1/13/2023.
Based on record review, the licensee did not comply with the section cited above, Infant # 1 does not have LIC 9227, Individual Safe Sleep Plan on file signed by the parents or the infant's authorized representatives which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to have the authorized representative filled out and signed (if needed) the LIC 9227 Infant Safe Sleep Plan and submit the plan of correction to CCL no later than 1/13/2023.
