DIAZ FAMILY CHILD CARE

Based on observation and interview , the licensee did not comply with the section cited above upon LPA's arrival, Licensee stated that there were 10 children present while the assistant stepped out brielfly to run an errand.which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The deficiency was corrected upon the arival of the assistant who had been gone for approximately 10 minutes based on the licensee's statement. LIcensee agreed to submit a written plan of correction on how to prevent the similar incident from occurring in the future. The plan shall include that licensee will comply with the capacity limitations required for a small family child care home license if the assistant is not present.

Based on record review, the licensee did not comply with the section cited above Infant no. 1 who started today, 8/4/2025 does not have the record on file (enrollment packet) which poses/posed a potential health, safety or personal rights risk to persons in care. Provider stated that Infant one was not scheduled tostart today, but since mom had an emergency, child was dropped off without submitting any documents.

Resolution: Licensee agrred to submit a plan of correction no later than 8/14/2025. POC letter shall include how licensee will ensure that all required documentation is submitted prior to admitting a child in the FCCH

Based on record review he licensee did not comply with the section cited above two (2) out of two (2) infants do not have LIC 9227 infant Safe Sleep Plan on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to submit a plan of correciton no later than 8/4/2025, POC letter shall outlie how licensee will ensure that the Infant Safe Sleep Plan for infants 0 to 12 months will be completed by the parents. The plan includes whether the infant is able to roll back and forth.
