Based on observation, the licensee did not comply with the section cited above in infant child was observed in a car seat within a play-pen restricting infants movement which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee during the inspection immediately removed child out of the car seat, as well as removed the car seat from the play-pen.
Based on observation & interview, the licensee did not comply with the section cited above in 4 out of 4 infant children did not have an infant sleep check form documenting every 15 minute checks which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee was informed to begin documenting infant safe sleep checks every 15 minutes the next time an infant falls asleep
Based on observation & record review, the licensee did not comply with the section cited above in assistant did not have a current Mandated reporter training certificate which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Assistant is to complete the Mandated reporter training and to submit certificate to LPAs email: Fernando.Hernandez@dss.ca.gov

Resolution: Licensee will submit a copy of assistants immunizations to LPAs email (Fernando.Hernandez@dss.ca.gov)
