Based on record review, the licensee did not comply with the section cited above in that her Mandated Reporter Training certificate expired on 5/26/2023, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit proof of completion of AB1207 compliant Mandated Reporter Training to the Department (seena.parsapour@dss.ca.gov) no later than 9/10/2025.
Based on observation, interview and record review, the licensee did not comply with the section cited above in the provider shall physically check on the infant every 15 minutes, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit a completed safe sleep log for C1, to CCLD by 6/30/23, via email: Martina.Jimenez@dss.ca.gov
