Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to child in care. LPA observed an assistant (S3) alone in the facility providing care to 3 children without a current Pediatric First Aid/CPR certificate, which expired on 02/16/2025.

Resolution: Licensee to write a statement acknowleding how they will ensure qualified staff are present in the home providing care and supervision to children in order to comply with the above regulation. If S3 is to be left alone with the children, a current and valid Pediatric First Aid/CPR certificate for S3 must be provided to the Department of Social Services. Licensee to submit the statement and certificate via LPA email provided no later than 11/06/2025.

Licensee stated an Adult (A1) individual resides in the attached garage converted studio that is part of the facility's address. Licensee stated A1 moved into the studio in July 2025. LPA's review of Guardian on 10/23/2025 revealed A1's name was not associated with the licensed facility.

Resolution: Licensee submitted the LIC9181 Criminal Background Clearance Transfer Request to LPA Billones on 10/23/2025.
