Based on observation, the licensee did not comply with the section cited above in that LPA observed one infant sleeping in a swing and one infant sleeping in a playpen that had a large pillow and blanket in the crib which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to write a statement detailing how Licensee will follow infant safe sleep regulations and will submit statement via email to Francisca.Velazquez@dss.ca.gov by 8/11/2023.

Based on observation and record review, the licensee did not comply with the section cited above in that licensee is not completing LIC9227 for all infants under the age of 1 year which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have all famlies with children under the age of 1 year, complete LIC9227 and submit proof via email Francisca.Velazquez@dss.ca.gov by 8/17/2023.

At 12:07 PM, LPA observed an infant asleep in the bassinet at the time of the inspection. LPA observed three (3) infants in care with one(1) pacNplay and a one (1) bassinet, which poses/posed a potential health, safety or personal rights risk to persons in care.

Assistant (A1) has not completed the Mandated Reporter Training required per AB 1207, which poses/posed a potential health, safety or personal rights risk to persons in care.

LPA observed in the kitchen a drawer unsecured with knives accessible to children in care, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will email photos of the knives inaccessible to children in care by July 13, 2022.

LPA interviewed licensee and Ms. Hernandez-Medel, who stated during the interviews it was confirmed that Maria Socorro Hernandez -Medel, assistant has been working in the day-care since July 11, 2022. Ms. Hernandez-Medel does not have a criminal record clearance to assist in the home, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Ms. Maria Socorro Hernandez-Medel must get a criminal record clearance and submit all documents to the Community Care Licensing (CCL) by July 13, 2022. A $200 civil penalty was assessed

LPA observed a chicken in the children's playhouse. LPA observed a day-care child (C2) run to the playhouse and tell (C3) not to go into the playhouse. Licensee advised LPA the children are not allowed in the playhouse because the chicken may pic the children, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee secured the chicken during the inspection in a portable cage. Licensee stated will be making a permanent cage for the chicken by 7/19/2022. Licensee will email photos to CCLD of completed cage by 7/19/2022
