Based on interview and record review, the licensee did not comply with the section cited above in one out of three children's files were not available for review which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will review all children’s files and ensure that each file contains all required documents, including emergency contact information, immunization records, and signed consent forms. The licensee will submit proof of correction to the Department by providing copies or verification that all files have been updated. The licensee will also implement a system to review files regularly and update them as needed to maintain compliance. Licensee to submit proof to elizabeth.george@dss.ca.gov no later than January 23, 2026.
Based on observation, the licensee did not comply with the section cited above in the FCCH does not have a working smoke detector in the home which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will immediately install operational smoke detectors in the FCCH. The licensee will submit proof of correction to the Department, such as photographs of installed smoke detectors via email at elizabeth.george@dss.ca.gov no later than January 23, 2026.
Based on interview and record review, the licensee did not comply with the section cited above in Fire Drills are not being conducted or documented which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will immediately implement a schedule to conduct fire and disaster drills at least once every six months, as required. The licensee will maintain written documentation of each drill, including the date, time, and type of drill conducted. A copy of the updated fire drill log will be submitted to the Department as proof of compliance to elizabeth.george@dss.ca.gov no later than January 23, 2026.
Based on record review, the licensee did not comply with the section cited above in daily infant sleep logs are not being documented when infants are in licensee care which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will immediately resume updating infant sleep logs daily for each infant in care. The licensee will implement a system to ensure ongoing compliance, such as using a daily checklist or digital tracking tool. A copy of updated sleep logs will be submitted to the Department as proof of correction to elizabeth.george@dss.ca.gov no later than January 23, 2026.
Based on record review, the licensee did not comply with the section cited above in both licensee and assistants CPR and First Aid certification expired November 2025 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will complete an approved CPR and First Aid training course and obtain a valid certification. The licensee will submit a copy of the renewed CPR and First Aid certifications to the Department as proof of compliance. The licensee will also ensure that certifications are renewed prior to expiration in the future. Proof of completion to be submitted to elizabeth.george@dss.ca.gov no later than January 23, 2026.
Based on observation, the licensee did not comply with the section cited above in two bouncers, one bumbo without a safety belt and two fisher price rock n play sleepers were located in the play room and accessible to children in care which posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee removed all prohibited items to an off-limits and inaccessible area of the FCCH during the inspection. The licensee will submit a written statement affirming that all items not permitted in a Family Child Care Home will remain inaccessible and that the facility will adhere to all applicable regulations regarding prohibited equipment. This will submitted via email to elizabeth.george@dss.ca.gov no later than January 23, 2026.
Based on observation, record review and interview, the licensee did not comply with the section cited above in not documenting safe sleep checks for the infant they care for, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide documentation of safe sleep checks for the next 2 weeks. Licensee will provide the documentation to LPA via email at giovani.gonzalez@dss.ca.gov
Based on record review and interview, the licensee did not comply with the section cited above in neither themselves or assistant having a current Mandated Reporter Training certificate which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and assistant will provide an updated Mandated Reporter Training(AB 1207) to LPA via email at giovani.gonzalez@dss.ca.gov no later than 11/06/2024.
Based on record review, the licensee did not comply with the section cited above in 3 out of 6 children's file not being available for review which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide file for the children that were present during the inspection. Licensee shall provide files to LPA via email at giovani.gonzalez@dss.ca.gov.
Based on observation the licensee did not comply with the section cited above in having cleaning products stored in unlocked cabinets and placed on an accessible counter which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: During the inspection objects were removed from accessible areas. Licensee will provide a written statement to LPA on how they will ensure cleaning products are not accessible to children in care no later than 11/06/2024 via email at giovani.gonzalez@dss.ca.gov
