Based on interviewand record review, the licensee did not comply with the section cited above in 2 out of 2 count in the licensee could not provide immunization for her self and S1 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send confirmation of updated immunization to LPA. Bussiness card with contact information provided. Licensee will submit a POC describing what the licensee will implement to stay current with immunization.

Based on LPA observed a carbon monoxide detector that was not functioning at the time of the inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.
