Based on observation/ interview/ record review, the Licensee did not comply with the section cited above as the LIcensee and Assistant (A1) do not have current certifications for Mandated Reporter training, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee to provided CCLD (elvin.baddley@dss.ca.gov) current Mandated Reporter certifications for Licensee and Assistant (A1) no later than the closed of business (5:00 PM) on 8/27/24.
Based on record review the licensee did not comply with the section cited above inLicensee did not renew the Mandated Reporter training whihc expired on 10/29/2020 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to take the Mandated reporter training and submit the prrof to CCL no later than 8/5/2022.
Based on observation, the licensee did not comply with the section cited above the fire extinguisher was last serviced on 8/6/2019 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to service or buy a new fire extinguisher and submit proof to Community Care Licensing (CCL) no later than August 5, 2022
Based on interview and record review, the licensee did not comply with the section cited above in Licensee was not documenting the napping infant. which poses/posed a potential health, safety or personal rights risk to persons in care. Licensee stated infant was not napping at all times. Licensee also stated she was not well versedof the Safe Sleep Regulation
Resolution: Licensee agreed to review the copy of Safe Sleep Regulation provided by LPA and log the napping infants every 15 minutes.
