the Notice of Site Visit from the Department's last visit to the Facility on June 4, 2024 was not posted upon LPA's arrival to the Facility.
Based on observation, there was not a cover on the electrical outlet near the entrance door that has the stairs. This poses a safety hazard to children in care.
the Facility was operating out of ratio when one partially qualified teacher opened the Facility on the following dates: 05/14/24, 05/16/24, 05/21/24, 05/23/24 and was the only staff present with anywhere from one to six children present during the early morning session. This presents a potential risk to the health, safety, or personal rights of children in care.
