After water testing was completed at the facility, Faucet C had an ALE of 8.9, which posed a potential health, safety or personal rights risk to persons in care.

The Licensee did not notify the Department of the unusual incident until written report was received 9/5/2023, twelve days after the incident occurred, which poses a potential threat to the health, safety, and personal rights of children in care.

(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents.

(g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children.

The Licensee did not ensure that children were visually supervised at all times as a child was left unattended on the playstructure, while other children (unaffiliated with CKC) were on the playstructure, which poses an immediate risk to the health, safety, and personal rights of children in care.
