Resolution: Licensee stated she is currently enrolled to take the Pediatric First Aid/CPR on Saturday 11/16/24. Licesee stated, she will submit certification by due date.
Based on record review, the licensee did not comply with the section cited above in having expired (4/3/2024) Child Abuse Mandated Reporter Certificate which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated, she will go online to retake the Child Abuse Mandated Reporter training and submit proof by due date.
Based on interview and record review, the licensee did not comply with the section cited above in have staff #1 present and working with children for more than a month without a criminal records clearance which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee informed staff #1 cannot be present and must leave and cannot return until the facility receives the criminal record clearance.

Resolution: Per licensing she will conduct and emergency drill and document drill before the due date.

Resolution: Licensee stated will begin documenting the 15 min sleep logs for children 0-2 by due date and keep them in their file.
Based on LPA record review and licensee statement, the licensee did not comply with the section cited above in not completing LIC 9040 Child Care Facility Roster which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated will complete LIC 9040 Child Care Facility Roster and will keep document for 3 years.
