Based on observation, the licensee did not comply with the section cited above because the crib sheet used for one infant (C1) was not appropriately sized for the mattress, did not fit tightly, and was dislodged which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee to replace the crib sheet by the POC due date with one that is appropriately sized for the mattress, fits tightly, and overlaps the underside of the mattress to prevent it from becoming dislodged. Licensee to submit a picutre of crib with the new sheet inside by the POC due date.
Based on records review, the licensee did not comply with the section cited above because one assistant (S1) does not have immunization records of measles, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee to provide S1's measles record by the POC due date.
