Based on observation and record review, the licensee did not comply with the section cited above, LPA did not observe a complete file for S2 file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete a file for S2, this is to include a Health Screening(LIC503) with TB, and send a copy to CCL(Community Care Licensing)by the POC date, 5/14/25.

Based on observation,interview, record review, the licensee did not comply with the section cited above, S2 does not have copies of Immunizations in file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will send copy of Immunizations to CCL by the POC date.

Based on observation,interview and record review, the licensee did not comply with the section cited above, C2 does not have a Individual Infant Sleeping Plan in file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a copy of the LIC9227(Individual Infant Sleeping Plan) for C2 to CCL(Community Care Licensing)by the POC date.

Based on record review, the licensee did not comply with the section cited above by not having current Mandated Reporting training done for S1, S2, and S3 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to complete the mandated reporters training for S1, S2, and S3 and to submit a copy as proof to CCL by POC due date.

Based on record review, the licensee did not comply with the section cited above by not conducting 15-minute sleep checks for C1, C2, C3, and C5 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to create a 15-minute sleep log for C1, C2, C3, and C5, and to submit proof of sleep logs to CCL by POC due date.
