Based on record review, the licensee did not comply with the section cited above in S2. Staff did not have evidence of a current TB test in S2's file which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above two infants ( #1 and #5). Licensee stated that she observes the infants sleeping, but has not documented the 15 minute checks, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will implement a 15 minute check for all sleeping infants up to age 2 and send a copy to CCL by the POC date. The form must note that every 15 minutes, the provider checked the infant for labored breathing, signs of distress, that infants up to 12 months of age are sleeping on their back, the infants name,date and time of the check

Based on record review, the licensee did not comply with the section cited above, Licensee, S1, S2 S3 don't have a curent Mandated Reporter Training on file, in which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above, S2 does not have current immunizations on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will submit a copy to the immunizations to CCL by the POC date.
