Based on interview and record review, the licensee did not comply with the section cited above. Licensee has not documented 15 min sleep checks for C1-C4, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a plan to ensure that she will document the 15 minute checks by the POC date

Based on interview and record review, the licensee did not comply with the section cited above. S1 began employment in October 2025 and does not have fingerprint clearances, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: By POC due date, 2/13/26, licensee to submit a written plan to ensure that all adults working in the home shall have fingerprint clearances prior to initial presence in the home. Civil Penaly assessed of $500.00 was assessed.

Based on observation, the licensee did not comply with the section cited above. LPA observed that the bedroom door was not open when two of the infants were sleeping, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a plan to ensure that the door remain open while infants are sleeping by the POC date.

Based on interview and record review, the licensee did not comply with the section cited above, Licensee did have current documentation for the 15 min checks while the infants(up to two) the infants sleep, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will continue to document the 15 minute checks for the infants. Licensee will submit a plan to ensure that she will document the 15 minute checks by the POC date

Based on observation, the licensee did not comply with the section cited above. LPA observed the two doors closed where C3 and C5 where sleeping. Staff were not present in the rooms or not stationed in the same area, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a plan that will address that when infants are sleeping in a seperate room from where the provider is stationed, the doors to the rooms where the infants are sleeping shall remain open at all times, by the POC date.
