Based on observation,interview, record review, the licensee did not comply with the section cited above. Licensee stopped documenting C1's 15 minute sleep checks when C1 turned 12 months old, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they will begin documenting C1's sleep checks begining 2/10/24. Licensee will submit a plan stating that they will Safe Sleep regulations.
Based on interview the licensee did not comply with the section cited above. Licensee stated that they have not documented the 15 minute checks while the infants are sleeping, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit a plan to ensure that the 15 minutes checks are being conducted for the infants in care.
Based on observation, the licensee did not comply with the section cited. LPA observed C5's pacifier had an attachment while sleeping. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will no longer allow C5 or any other infants who use pacifiers have any attachments while they sleep. Licensee stated that she will submit a plan to ensure that this will not occur again.
Based on observation, the licensee did not comply with the section cited above. LPA observed C4 sleeping with a stuffed animal. which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will submit a plan to ensure that none of the infants sleep with any items/stuffed animals.
Based on record review, the licensee did not comply with the section cited above: LPA observed in C4's file that a LIC9227 was missing, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that they will submit a complete LIC9227 to CCL by the POC date
