Based on record review and interview, the licensee did not comply with the section cited above. Licensee's Mandated Reporter Training expired in 2025, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a copy of current Mandated Reporter Training to CCL by the POC date.

Based on interview, and record review the licensee did not comply with the section cited above with C1-C3 15 minute sleep checks were not being documented while in care, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will submit a plan to ensure that she continues to document the 15 minute checks for infants (up to 24 months) in care by the the POC date.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee does not have an Individual Infant Sleeping Plan (LIC 9227) for C1,which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a copy of a complete LIC9227 to CCL by the POC Date.

Based on interview and record review, the licensee did not comply with the section cited above. LPA observed that the Licensee has not been documenting the 15 minutes checks for three infants (C1-C3), which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will submit a plan to ensure that she will remain in compliance in Infant Safe Sleep regulations and submit it to CCL by the POC date

Based on interview, the licensee did not comply with the section cited above. Licensee's Pediatric First Aid/CPR expired. Licensee stated that it has expired, but did not know the exact date(LPA was unable to observe a copy), however,was unable to which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will submit a current copy of her Pediatric First Aid/CPR to CCL by the POC date.

Based on interview, the licensee did not comply with the section cited above . Licensee's Mandated Reporter Training expired in 1/23/2022, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will send a copy of the Mandated Reporter Training certificate by the POC date.

Based on interview, the licensee did not comply with the section cited above. Licensee stated that she is supervising the children while they are sleeping, however, she has not documented the required 15 minute checks for the C2 and C4, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will note/document that every 15 minutes, she checked the infant for labored breathing and signs of distress. and send a copy to CCL by the POC date.

Based on record review, the licensee did not comply with the section cited above. LPA observed that the Licensee's First Aid/CPR expired on 10/2021 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will send a copy of her current FIRST AID/CPR to CCL by the POC date.
