Based on observation, the licensee did not comply with the section cited above. LPA observed adult scissors on a children's table in the day care room and a knife on a patio table near in the backyard playground accessible to children which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed during visit. Deficiency cleared today.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee's daughter Nekone Garay was present in the home for at least five days from 05/03/2024 through 09/21/2024 without fingerprint clearance which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a letter stating she understands the above regulation and will have her daughter fingerprint cleared prior to being present in the home to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above. LPA observed an infant sitting in a high chair not being fed which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a statement stating she understands she may not detain children in high chairs and that she will only place them in their high chairs during feeding time.

Based on observation, interview and record review, the licensee did not comply with the section cited above. LPA observed room additions including a day care room and bathroom in the home. Licensee failed to inform the Department of changes which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit and updated sketch to CCLD by POC date. Licensee was also informed these additions to the home may not be used until a fire clearance is obtained

Based on record review, the licensee did not comply with the section cited above. Child 2, 3, and 5 immunization records need to be updated which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit updated immunization records for child 2, 3, and 5 to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Child 2 is missing LIC627 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC627 for child 2 to CCLD by POC date.

Based on interview and record review, the licensee did not comply with the section cited above. PA observed licensee is not completing Infant Safe Sleep log properly and only documenting the date and time they sleep which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will document a safe sleep log for all infants in care beginning 09/30/2024 through 10/11/2024 to CCLD by POC date.
