Based on observation, interview and staff roster review, the licensee did not comply with the section cited for one staff (S4) which poses a potential health, safety or personal rights risk to persons in care. DIrector failed to transfer staff fingerprints.

Based on observation, interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. The director packet was not submitted to the Childcare Licensing Department within 10 days of a change of director.

A child was left unattended inside Preschool Room Two on February 29,2024. This poses an immediate risk to the health, safety, and personal rights to children in care.

Food Services:(7) Modified diets prescribed by a child's physician as a medical necessity shall be provided. (B) A child shall not be served any food to which the child's record indicates he/she has an allergy. This was not met by: A child who has an allergy to dairy was given meal with diary which caused an allergic reaction.This poses a potential risk to the Health, Safety, or Personal Rights of children in care.
