Based on record review and interview, the licensee did not comply with the section cited above. Child #1 (C1) was observed to not have any immunization records. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated if C1's parents refuse to give child immunization, C1 will no longer attend the child care effective 03/26/2025. If C1 gets immunizations, Licensee will provide a copy of immunization records. Licensee stated she will provide LPA with update by POC due date.
