Based on observation, interview, and record review, the licensee did not comply with the section cited above in that 1 out of 1 persons did not have a criminal record clearance, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Person 1 left the facility during the inspection.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in 3 out of 3 assistants did not have proof of immunizations for MMR, Tdap, and tuberculosis which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will submit proof of immunizations for 3 assistants by June 14, 2024.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in 5 out of 5 infants did not have a Sleep Log on file for review, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee stated they will submit Sleep Logs for 5 infants by 5/20/24.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in 3 out of 3 assistants did not have current Mandated Reporter Training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will send proof of completed Mandated Reporter Training by 6/14/24.

Based on observation, record review, During file record reviews licensee’s mandated reporter training certificate was expired. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, they were closed for over 1 year during COVID-19 so they didn't realize the mandated reporter training certificate was expired. They will go on the website and renew the training. Licensee will submit a copy of the certificate to licensing department by the due date. www.mandatedreporterca.com

Based on observation, record review, During children file reviews child #1 and child #2 were missing LIC9227 Safe Sleeping plan. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will give the form LIC9227 safe sleep plan to parent's of C1 and C2 to complete. Licensee will submit a copy of the forms completed to licensing department by the due date.
