Based on observation, interview and record review, the licensee did not comply with the section cited above in LPA obsevered one infant child sleeping inside a playyard with a pillow and blanket inside which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed the pillow and blanket from the child's playyard and stated she will no longer put loose articles/objects in the playyards.

Based on observation, interview and record review, the licensee did not comply with the section cited above in LPA observed one blanket hanging on one side of the playyard. LPA also observed a pocket attached to a side of the playyard which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed the blanket and pocket from the child's playyard and stated she will no longer hang anything on the side of the playyard and will not have anything attached to the side of the playyard.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in an emergency drill was not conducted in the last six months, the last drill was conducted on 4/30/2023, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee, will email/send LPA Valdez Santana proof of documented emergency drill by POC due date.

Based on observation, interview, record review, At 3:00pm during children’s files reviews, child #1 and child #2 did not have LIC9227 Individual Infant Sleeping Plan in their files. Licensee stated they were not aware of the form LIC9227. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they will provide LIC9227 form to parents of C1 and C2 to complete. Licensee will send a copy of the documentation to licensing office by the due date.

Based on observation, interview, Upon arrival at 2:30pm LPA Odom observed child #2 sleeping on a baby bounce, licensee stated C2 was fuzzy most of the day and they had just fallen asleep on the bouncer. The licensee did not comply with the section cited above in which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, they will get rid of the bouncer, donate the bounce and no longer use the bouncer for the infant. Licensee immediately removed the infant from the bouncer and placed C2 in the playpen. Licensee will send a written statement with plan of correction to make sure they will no longer utilize a bouncer in the childcare home. Licensee will submit POC to licensing by the due date.

Based on observation, interview, record review, Licensee did not have a napping log for C1 and C2 to document every 15 minutes when the infants are napping. Licensee stated they were not aware of the napping log. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, they will begin to document on the napping log for C1 and C2 every 15 minutes when they are napping. LPA provided a sample of the napping log and explained the form. Licensee will submit a copy of the napping log for C1 and C2 to licensing office by the due date.

Based on observation, interview, record review, At 3:30pm, during staff file review licensee’s mandated reporter training certificate was not available for review the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated they completed the training a couple years ago but they have not renewed the training and they could not find a copy of the certificaet. LPA Odom provided the website to complete the training online, www.mandatedreporterca.com Licensee will submit a copy of the certificate to licensing office by the due date.
