Based on interview and record review, the licensee did not comply with the section cited above. Staff #2 did not have a current mandated reporter certificate on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will have Staff #2 complete the training and will send LPA the certificate by POC due date.

Based on interview and record review, the licensee did not comply with the section cited above. 2 out of 2 infants did not have 15 minute sleep logs in their file. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will complete sleep logs for 2 of 2 infants and will submit proof to LPA by POC due date.

Based on observation and record review, the licensee did not comply with the section cited above. 1 out of 2 infants did not have the LIC 9227 form complete. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will have parents complete the LIC 9227 form and will place in infant's file.

Based on observation, interview, record review, During staff file review licensee and assistant’s mandated reporter had expired on 10/2022. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review, the licensee did not conduct a disaster and fire drill in over 6 months. Licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, they will conduct a disaster and fire drill this week and submit a copy of the fire drill log to licensing department.

Based on observation, interview, and record review, Child #1 did not have an Infant Sleeping plan in file. The licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, they will have child #1 parents complete the LIC9227 form by this week. LPA Odom provided a copy of the LIC9227 form on today's visit. Licensee will submit a copy of the completed form to licensing department by due date above.

Based on observation, interview, and record review, the licensee failed to create and document a sleeping log for 4 infants in care. Licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated, they will create a sleeping log and begin to document every 15 minutes for all infants under the age of 24 months old. LPA Odom provided a sleeping log sample and explained the specific details of what needs to be documented on the sleeping log for each child and it needs to be filed in each child's files. Licensee will provide a copy of the sleeping log for each child to licensing department by due date above.
