Resolution: Licensee stated, they will have child #1 parents complete the LIC9227 form by this week. LPA Odom provided a copy of the LIC9227 form on today's visit. Licensee will submit a copy of the completed form to licensing department by due date above.

Resolution: Licensee stated, they will create a sleeping log and begin to document every 15 minutes for all infants under the age of 24 months old. LPA Odom provided a sleeping log sample and explained the specific details of what needs to be documented on the sleeping log for each child and it needs to be filed in each child's files. Licensee will provide a copy of the sleeping log for each child to licensing department by due date above.
