Based on observation, interview, record review, at 2:30pm during staff file reviews, 1 staff out of 4 staff did not have their immunization records available for review. Licensee stated, they received a copy, but they misplaced the records. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated, they have spoken staff member on today's date asking them for a copy of their immunization records, staff will obtain a copy and provide the copy to licensee. Licensee will submit a copy of the document to licensing department by the due date.
